Creating/Editing a Form Template

To create a new form template, head to the Categories/Templates Page - via links available on the Left Navigation Bar, and navigate to the Form Templates Tab.

Click on the + Create Form Template button to open the Form Template Popup.

Form Template type is pre-selected on creation, but can be modified using the dropdown on the top-right corner of the popup. A name for the template can be provided in the Description field.

The form can then be populated with form items using the Item Selection Section on the left side of the popup. Items can also be filtered by type using the filter atop the Items Table.

To add an item to the form, click on the item row to select it, and then click on the Add button that appears on the right corner of the item row. Alternately, double clicking an item also adds it to the form.

Newly added items will appear on the right side of the popup, temporarily appearing blue, before returning to white. Hovering over the item displays two action buttons on the right corner of the item.

The red x button removes the item from the form. The = button allows dragging the item up and down to sort them in the form.

A blue colored button on the top right side of the selected items table allows viewing a preview of the form that will be built.

Once information is provided for the form, click Save to submit information and have it stored into our database.

To edit an existing form, click on a form template row to open it in the Form Template Popup. Modify any required information and click Save to submit information and have it stored into our database.

A warning prompt will be shown when updating an existing Form Template, informing user of the scale of impact of their changes. Confirming this prompt will update the form template.

Cloning a Template

To clone a template, head to the Form Templates Page and right click on a Form Template Row to open the action menu. Clicking on the Clone Template option opens up the Form Template Creation Popup, with information cloned from the previous template. User can choose to modify any information needed, and click Save to create a new template with cloned information.
